They were discontinued many years ago, but you can still find replacement parts for them.

As an owner of a six-pack of them, I concur, that's a 525, probably an SS model (but could be PC for "plated chrome").
fwiw, there's a good thread that shows the differences between the 525 and the 630 that superseded it...
Perlick 525SS compared to new 630SSI just got three 630SS from Perlick to replace my 525SS faucets that I have had lots of leaking issues with. I held out until the redesign was out because I didn't want the 408 rear sealing faucet that they were offering before. I see someone else posted a thread with info on the 630 but...
